Quick Summary
This help article will guide through the donation form step by step and explain the different form fields and how they function. Additional features on the form field will be explained, too.
Personal Data
Title/Salutation: A customizable drop down menu with different forms of salutations.
The most common are: Mr., Mrs. or Family.
This field can be hidden, if desired.
Title: A customizable drop down menu to choose an academic title.
The most common are: Dr., M.A. etc.
This field can be hidden, if desired.
First name
Last name
Date of birth
Occupation: A customizable drop down menu to choose a form of occupation from.
The most common are: Worker, Employee, Self-employed etc.
This field can be hidden, if desired.
Address
Search address: Enter a street name, options of the street you are searching for will appear, choose the correct one and the rest of the address brackets (street, postcode & city) will auto-complete themselves, the exception being the house-number.
In case your signal is not strong enough, you can manually enter all address details.
The Country is set beforehand when the campaign is set up, however it is possible to change it. The preset country can of course also be changed afterwards.
If desired, we can show a field for additional address details.
Contact data
E-mail: Fill in the donors e-mail-address. The welcome e-mail will be sent to this address right after completing the donation.
Phone: Insert landline phone numbers here.
Mobile: Insert mobile phone numbers here.
Each of these fields can be a mandatory field. It is also possible to make one phone number (either landline or mobile) mandatory.
Additionally, we can insert an Opt-In-field under the e-mail and/or the phone number fields.
Welcome-SMS: If desired, you can add a button to send welcome-SMS while filling in the form and to check if the mobile number was entered correctly. You find more information about this feature in the dedicated help article: Welcome-SMS-Feature
Sponsorships/donation details
If you choose to show different sponsorships on the donor form, you have to choose one sponsorship. Note that different sponsorships can have different minimum donation amounts.
Independent from sponsorships, the next step is...
- ...the selection of a donation proposal
- ...the input of a monthly contribution in the field "donate different monthly amount"
- ...the input of a yearly contribution in the field "donate different yearly amount"
In all cases, the last two fields, will fill in automatically. If you insert 10£ in the monthly field, the yearly field will automatically change to 120£.
Now you just have to choose, in which interval the donation should be withdrawn. Note, that every interval can have different minimum contributions, that have to be defined during the campaign setup. Additionally, you can choose an interval, that is preselected on the form.
There are the following withdrawal intervals you can choose from: Annual, biannual, quarterly, monthly and single payment.
Bank data
Payment method: Here you select the desired payment method. The possible methods are defined during the campaign setup. You can also define a preselected payment method.
There are the following payment methods:
Country: Same as the address field, the country is preselected, but can be edited manually. The accepted bank countries are defined during the campaign setup.
Bank account holder: This field fills in automatically the first name and the last name entered in the first section of the form. Is the bank account holder an other person, the name can of course be changed manually.
BIC/bank code: When a correct bank code/BIC is entered, the field "bank name" is filled automatically.
IBAN/account number: The software checks, if the IBAN is correct and warns you, if that's not the case.
Bank name: Is filled automatically, after you entered a correct BIC or bank code.
When you choose to show the payment method "skip bank data" and you select it, all fields related to bank data disappear and you can finish and upload the donation without entering any bank details. However, if done so, it is not stored unter donations, but under "unfinished donations". After the bank data is filled in, the data set will be saved in donations.

Additional form fields
In addition to the standard form fields described above, you can insert as many customisable form fields as you want. To learn more about additional form fields, read the dedicated help article: Additional form fields
When you filled in the form completely, press on "Next", to get to the feedback-page.
The feedback-page
The second page of the form is completely optional and the feedback questions are fully customisable. The questions can be defined as mandatory or not.
After completing the feedback questions, click on "Next", to get to the summary/signature page.
The summary/signature page
The last page of the donation form is a summary of all data entered, so that the donor can check if his data was entered correctly.
Signature
Beneath a customizable disclaimer the new donor can put his signature by using his finger.
By clicking the "Clear" button on the bottom left corner of the signature field, the donor can clear his signature and try again.
Finish donation
If everything is correct, click the "Donate" button to upload the mandate. After uploading you will either receive a notification that the upload was successful or that the internet connection is not strong enough and the competed mandate is saved in the cache. If the later one is the case just go back to your dashboard when the internet connection is strong enough and click on the button on the bottom of the page to upload your mandate.
To get back to the dashboard, click on the button "Back to dashboard" in the bottom left corner of the form.
To clear all data entered, click on the red "Cancel" button at the bottom of the form. Now you are at the beginning of the form again and all data is cleared.
Note
It is possible, that you see fields, checkboxes, etc. on your donation form, that are not explained in this article. The reason is, that we set up every campaign individually and they are very customisable. This is also the reason for different wording of certain fields. If you have any problem with any fields not described here, or other difficulties with the form, just write to our support team (support@formunauts.com).
Other features
Amounts per age group
This feature aims to display the appropriate contributions for each age group, therefore the contribution suggestions are only displayed after the date of birth has been entered
This means, for example, that 20-25 year olds see the suggested amounts 10,15,20,25.
While 26-30 year olds see the suggested amounts 20,25,30,35.
You can define up to 4 suggested amounts per interval and per age group.
If the age groups are defined correctly, then realizations can be demonstrably increased by up to 7%.
Fundraiser swapping
If Fundraiser Swapping is enabled for your company/organisation you need a team leader account to switch between the different fundraisers on the team. Simply access the donation form while being signed in as team leader and click on the fundraisers profile picture in the top right corner.
A drop-down menu will appear to show you the fundraisers in your team, select the desired fundraiser profile and now all donations will count towards this fundraisers account.
Switch between languages
A click on the "option" symbol left to your profile picture (small gearwheel) lets you change between the languages, that are activated for this campaign. The whole form and also the Welcome E-Mail are translated into the selected language.
Change of the font size
Clicking on the "option" symbol also lets you change the font size on the form between small, medium and large.
